Providence Physician Group accepts most commercial health plans available in the Puget Sound Region. We bill most insurance plans directly. It is the patient’s obligation to provide the insurance information and notify the clinic of any insurance changes. It is important that you know and understand your insurance plan so you aren’t surprised by unexpected charges. As each plan varies in coverage, it is important to find out what out-of-pocket expenses you can expect. Please familiarize yourself with any restrictions regarding self-referrals and find out about co-payment policies.
If your health plan requires a co-payment for office visits, please plan to pay the receptionist before you see your doctor or other primary care provider. If you have any questions about your co-payment requirements, please contact your health insurance company before your visit.

If you are not covered by an insurance plan, we ask for a $75.00 deposit at the time of your visit. You will be billed for the remaining balance. If you are being evaluated and treated for injuries caused by a motor vehicle accident, it is your responsibility to inform the receptionist and physician upon arrival. You will also be responsible for payment of services relating to the accident at the time of your visit. We will provide you with documentation to submit to your auto insurance company or attorney for reimbursement.
