Home Health strives to help patients attain independence and manage their illness or condition
Under physician's direction, Home Health provides: - Skilled care by RNs - Medical social work (illness-related personal & family problems) - Home health aides (help with bathing, dressing, etc.) - Rehabilitation therapies (phys., occupational, & speech therapy) - Instruction & support for family and caregivers |

Payment for Home Health Services Medicare and most insurance companies cover home health services. Home Health can verify your benefits. Note: Medicare patients must be home bound to qualify.
How to Initiate Home Health Patients can be referred by their physicians, discharge planners or clinics.
A physician’s order is required to obtain home health services. However, you may request services directly from Providence Home Health. We will notify your physician about the services you will receive.
Physicians can also submit referrals via our fax order cover sheet and fax referral form. |
