Interdisciplinary Team Approach "Interdisciplinary Hospice Team" is the term used for a patient’s circle of caregivers. The Primary Physician – An important participant in this Interdisciplinary Team is the patient’s physician, who oversees the plan of care and acts as the "attending physician." A Medical Director - A medical director consults with the patient’s primary care physician as well as the nurses to oversee the coordination of care. Specially Trained Nurses - Nurses make regular in-home visits and there is a nurse on call 24 hours a day to answer questions and make a home visit if needed. Medical Social Workers – The emotional, psychological and social needs of the patient and family members are assessed by a social worker. Working with the family, they address such issues as communication, financial planning, community resources and patient advocacy. Home Health Aides – A certified Home Health Aide is available to assist patients with bathing and other personal care needs. Physical, Occupational and Speech Therapists – They are available to assist the patient and family in adapting to the home environment. Bereavement Counselors - A comprehensive bereavement support plan follows the family for one year after the death of a loved one. Volunteers – Our volunteers provide unparalleled support services to the patient and family. Some of the ways in which they provide assistance include running errands, offering relief for the caregiver, reading stories and other activities. Chaplains – Our Hospice chaplains offer spiritual and emotional comfort to patients and their families allowing a patient to meaningfully explore this experience. Funeral and memorial services can be arranged upon request.
