About the Senior and Community Services Division
The Providence Senior and Community Services Division seeks to further the mission and core values of the Providence Health System by continuing the healing ministry of Jesus in the world of today, with a special concern for those who are poor and vulnerable. Working with others in a spirit of loving service, we strive to meet the health care needs of people as they journey through life. The following facilities and programs comprise the Senior and Community Services Division:

Providence ElderPlace
A PACE (Program of All-Inclusive Care for the Elderly) program in south Seattle, Providence ElderPlace is an innovative model of frail elder care and the only PACE program in Washington. ElderPlace provides transportation to and from its facility each day, bringing participants and their care providers together. Participants receive a hot meal, socialize with staff and other participants, and meet with their doctors. Our comprehensive, high quality health and social services are tailored to meet the unique needs of each individual, assisting them in remaining as healthy and independent in the community for as long as possible. ElderPlace serves seniors who live throughout King County.

Heritage House at the Market
Established in 1990 at the Pike Place Market, Heritage House at the Market offers a quality urban lifestyle with the assisted living services that people want and need. Heritage House was the first assisted living community in Washington to serve a diverse population including the needy and disenfranchised. Apartments and common spaces have breathtaking views of Puget Sound and the Olympic Mountains. Heritage House is the result of a public/private partnership among Providence Health System, the Seattle Housing Authority, Pike Place Medical Clinic, City of Seattle, and State of Washington. Because of our mission and specialized services, the residents’ average length of stay is nearly double the national average in assisted living.

Providence Hospice of Seattle
Providence Hospice of Seattle was established in 1975 as the first hospice in the Northwest and the third in the U.S. We provide care for terminally-ill children and adults in King and south Snohomish counties. Hospice services are provided wherever the patient may reside—at home, in a skilled nursing facility, or residential care facility. Patients receive palliative care (reducing/eliminating pain and increasing comfort without curing), and with their families receive social, emotional, and spiritual support including bereavement counseling. Innovative programs provide support through difficult times. For example, Safe Crossings provides counseling for children losing a sibling or parent. Providence Hospice of Seattle is the largest hospice in King County.

Providence Infusion and Pharmacy Services
Providence Infusion and Pharmacy Services, established in 1993, is a stand-alone, non-hospital based pharmacy and patient care company.   State-of-the-art sterile compounding facilities enable Infusion to provide USP 797 (United States Pharmacopeia) compliant sterile products, which are shipped nationwide.  Intravenous and enteral therapies are provided to patients in their homes and in ambulatory settings.  Pharmacists, nurses and nutritionists develop integrated clinical care planning for each patient.   Home nursing services are provided to ensure therapy-specific patient education and training as well as to monitor and manage the patient’s care in the home setting.   Providence Infusion and Pharmacy Services is a state-licensed home health agency, a state-licensed pharmacy, and has been JCAHO accredited since 1995.

Providence Marianwood
Founded in 1986 in Issaquah, Providence Marianwood is the only not-for-profit skilled nursing and home health agency on the east side of King County. We fill important community needs by providing quality care to those most in need. We provide short-term care for rehabilitation and complex medical needs between a hospital stay and home, a secured Alzheimer’s unit, long term care with 24-hour skilled nursing care, and respite care. Our Faith in Action program offers free volunteer caregiving to those in need in the Issaquah, Sammamish Plateau, and Snoqualmie valley regions.

Providence Mount St. Vincent
Established in 1924 in West Seattle, Providence Mount St. Vincent is home to more than 400 adults during the transition from independence to increasing reliance on the support of others. Staff are committed to providing comprehensive care that respects the personal values and dignity of older adults and others with a variety of physical and cognitive abilities. Residents range in age from 29 to 106, with an average age of 89. We offer many innovative services such as skilled nursing in a home-like family environment; assisted living apartments; adult day health, rehabilitation services, a wellness clinic, beauty/barber shop, and gift shop for residents and the surrounding community; and an intergenerational learning center serving 125 children who interact with residents.

Providence Home Services
This program encompasses two programs that help people stay in their homes while getting the care they need. Providence Home Health provides skilled nurses and therapists to assist people going through a change in their health status to preserve their function and independence at home. Providence Private Duty Homecare helps people stay in their home environment by providing personal care and companion services to manage the activities of daily living (bathing, meal planning and preparation, transportation, Alzheimer’s care, and more). Both programs serve the King County community.
