Services
Our services are available to patients and families within a 30-mile radius of Providence Portland Medical Center and Providence St. Vincent Medical Center. Patients living outside this service area will be referred to a member of our provider network located closer to their home.
Insurance coverage
Medicare, Medicaid, Worker’s Compensation and most private insurance companies and HMOs cover the cost of home health care. Coverage and prior authorizations differ from plan to plan. Our staff will verify benefits and communicate any financial responsibilities that you may have.
Financial assistance may be available, based on income, if you do not have insurance.
Patient referrals
A physician’s order is required to obtain home health services. However, you may request services directly from Providence Home Health. We will notify your physician about the services you will receive.
Physicians can also submit referrals via our fax order cover sheet and fax referral form.
