Pre-register and Complete Hospital Forms

It is important for you to fill out two forms: 1) the hospital pre-registration form, and 2) the Clinical Admission Data document (CAD).

1. Complete and submit the online hospital pre-registration form at least 48 hours before the day of your surgery. If you have questions about this form, please contact us at 503-215-9565.
2. Complete and submit the online Clinical Assessment Document (CAD). This form gives us detailed information about your medical history and personal needs, helping us provide the best care for you. There are a variety of CADs to choose from. Please select the one that best describes your surgical plan:
   • Providence Portland Medical Center
   • Providence St. Vincent Medical Center
   • Providence Milwaukie Hospital
