Get the best possible care

To make sure you get the best, safest care possible, we need some important information about you. Before you come to the hospital for surgery, please complete and bring with you a copy of this medication history form and the health summary form. If the patient is a child aged 13 years or younger, the parent or guardian should complete the pediatric CAD instead of the health summary. Your safety and your health are important to us.

Providence Hospice Offers New Grief Support Class in Clackamas County

Journey Through Grief” is a six-session class designed to give an understanding of the grief process and to allow an opportunity to question, explore and share experiences that will lead to growth, recover and healing.

Art gallery enables creativity, healing

In collaboration with the Arts Action Alliance of Clackamas County, Providence Milwaukie Hospital adorns the collection of creative renderings of older adults, changing on a quarterly basis. Not only does the exhibit offer an opportunity for artists to showcase their work, it also emphasizes the role of art therapy in the healing process.

Making end-of-life decisions

Regardless of age, everyone should discuss their end-of-life wishes with family members and have documentation in place. If a person is unable to communicate desire for certain medical treatment, an advance directive can follow through with the individual's wishes.
