After your baby is born, the vital records technician at the hospital will give you a birth certificate worksheet. The birth certificate worksheet must be completed during your stay as an in-patient of the hospital and returned to the staff before you are discharged from the hospital. You will also receive a birth certificate order form. To obtain an official birth certificate for your baby, complete the order form and mail it to the county Vital Records office.
You may apply for a Social Security Number for your baby on the birth certificate worksheet. The hospital will send a request to the Oregon Health Division for a Social Security card to be ordered. Your baby's Social Security card will be sent to your mailing address within 4 to 6 weeks of your baby's birth.
