Things You Want Your Care Provider to Know

The birth of every baby is a unique and special event. If you tell us what is important to you in making this event the personal experience you want it to be, we will help you create the environment you want. For example, do you plan to use medications for pain control during labor? Do you want family members to attend the actual birth? If so, who do you want to have with you, and how do you want them to be involved?

To help you organize your thoughts, discuss requests and preferences, and explore options, we've listed some topics below. Talk with your partner and your family about these topics. Share your thoughts with your care provider and the Family Maternity Center nurses. We'll work to create a supportive, comfortable yet personal environment for your baby's birth.

You may want to print this page and write your thoughts down. Please feel free to share the completed sheet with your care provider.
