Manor resident The only in-house long-term care nursing home facility in Washington, Rosary Manor has 56 beds and a full-time staff of RNs, LPNs and NACs. Rehabilitation services are also available to assist patients in overcoming disabilities to reach their highest functional level in activities of daily life.

Rosary Manor provides convenient access to hospital facilities and needed services, and ease of care by attending physicians and clinical staff.

Rosary Manor has been an important part of the health care system in our community since 1965, offering a permanent home for those who need the 24-hour support of a skilled nursing facility, as well as a short-term option for patients who are stable enough to leave the hospital but not ready to go home.

Our New Home
Summer 1997 saw the exciting and long-awaited move of Rosary Manor from Chehalis to beautiful new facilities on the first floor of Providence Centralia Hospital. While our staff and their commitment to the highest quality care remains the same, the expanded 63-bed unit provides residents with more private rooms, bigger recreational, dining and visiting areas, and more convenient access to hospital facilities and needed services.

  • A full schedule of activities designed to meet the physical, social and emotional needs of our residents             
  • Professional social workers for resident and family support             
  • Pastoral care and religious services             
  • Assistance from our business office in handling financial matters. If you wish, we can make arrangements to have your Social Security, VA or other pension checks mailed directly to the facility, to be applied toward payment for care.

  • Transitional Care Programs Aid Speedy Recovery
    Specialized transitional care programs are designed to speed the recovery of patients who are stable enough to leave the hospital, but not yet ready to go home. While located at Providence Centralia Hospital, with access to its physicians, therapists and pharmacy, this service is delivered in Rosary Manor's more home-like setting at a considerably lower cost.

We help people who need specialty care for conditions such as:

  • Post-surgical conditions

Rehabilitation Program Restores Hope
Summer 1997 also saw the move of Rehabilitation Services from Chehalis to its new location at Providence Centralia Hospital. Here a physician-directed team of rehabilitation nurses, physical therapists, occupational therapists and speech-language pathologists, along with a social worker, clinical pathologist, dietitian, chaplain and consultants from other medical specialties as required, work together to help Rosary Manor's transitional and long-term-care residents achieve the highest level of functional skills and independence possible. No matter what type of therapy is required, we encourage residents and their families to become actively involved in setting and achieving personal rehabilitative goals. The rehabilitation experience can be one of both joy and accomplishment -- time when hope is restored.

Respite Care: Time Off for the Care Giver
Caring for a loved one at home can be challenging and exhausting. Sometimes you need a break -- a chance to refresh and relax. To help you meet this need, Rosary Manor offers short-term care designed to provide nursing and other necessary services for your loved one. We will work with you to make this temporary transition as pleasant as possible, while providing:

  • 24-hour skilled nursing services             
  • Recreation and social programming             
  • Nutritional programming and intervention

Insurance Coverage
Providence Rosary Manor is fully licensed and certified to offer coverage provided by Medicare, Medicaid and private insurance companies.
