Medical Records

Providence Archives does not hold current or recent medical records. All patient records are managed by the individual hospital or clinic at which service was provided. Access to medical records is governed by state and federal laws and by Providence Health & Services policy. The law also governs how long records are retained; generally, most medical records are destroyed after ten to twenty years.

If you have questions regarding your records, please contact your physician or the medical records department of the hospital or clinic where you were a patient. You will need to provide a signed, written request or release of information form. Medical records cannot be released to a third-party without authorization of the patient.

Adoption Records

Providence Archives does not hold any adoption case records.

Birth Records

Providence Archives does not hold official birth records. Birth records are usually available at the county in which the birth took place.

Student Records

Providence Archives is the official repository for student transcripts for Providence Academy, Vancouver, Washington; St. Joseph Academy, Yakima, Washington; and St. Thomas Home, Great Falls, Montana. For more information see Transcript Requests.

Transcripts for parochial schools in which the Sisters of Providence taught are maintained by the local parish or diocese. Archives staff can assist you in locating the appropriate repository.

School of Nursing Records

Providence Archives maintains the transcripts from Sacred Heart School of Nursing, Spokane, Washington. For more information see Transcript Requests. All other transcripts from the Providence Schools of Nursing are maintained by the department of licensing in the respective state. Please contact the Archives for more information.
