Transcript Requests

Providence Archives maintains student transcripts for:

• Providence Academy, Vancouver, Washington
• St. Joseph Academy, Sprague, Washington
• St. Joseph Academy, Yakima, Washington
• St. Thomas Home, Great Falls, Montana

To order an official copy of your transcript, please write to the Archives with the following information:

• Date of birth
• School attended
• Full name (maiden name, if applicable)
• Dates of attendance/graduation
• Address(es) of where to send the transcript(s)
• Signature

A service fee of $10 is charged for each official copy requested (plus $5 for each additional official copy).

• Providence Hospital (Oakland, California); 1904-1972
  Transcripts are serviced at the registrar’s office of Samuel Merritt University. For assistance call 510-869-1550.

Montana

• Columbus Hospital (Great Falls, Montana); 1894-1968
  Transcripts may be kept by the Montana Board of Nursing as part of the record of an active nurse. Files of deceased nurses are typically purged so likely do not exist in this office. Contact the board for more information at dlibsdnur@mt.gov or visit their website.
• St. Patrick Hospital (Missoula, Montana); 1906-1978
  Transcripts are maintained at the St. Patrick Hospital Learning Center. Contact the librarian for assistance: 406-329-5710.

Oregon

• St. Mary Hospital (Astoria, Oregon); 1905-1949
  To request a copy of transcript, contact Customer Service at the Oregon State Board of Nursing at oregon.bn.info@state.or.us or call 971-673-0685. Records are only kept for 75 years..
• Sacred Heart Hospital (Medford, Oregon); 1913-1933
  Records no longer exist; they were designated for destruction by the State of Oregon when they reached 75 years old.
• Providence Hospital (Portland, Oregon); 1944-1962
  To request a copy of transcript, contact Customer Service at the Oregon State Board of Nursing at oregon.bn.info@state.or.us or call 971-673-0685. Records are only kept for 75 years.
• St. Vincent Hospital (Portland, Oregon); 1892-1975
  For transcripts dating from 1907-1926, contact the University of Portland Archives at 503-943-7116. For transcripts dating from 1934-1975, contact the University of Portland registrar at 503-943-7321.

Washington

Application files for registered nurses are available through the State of Washington Public Disclosure Department of the Department of Health. To access the application file, email pdrc@doh.wa.gov or call 360-236-4836. Contact the Washington State Department of Health, Nursing Care Quality Assurance Commission at nursing@doh.wa.gov or call 360-236-4700 for general information. Records are only kept for 75 years.
