We believe that when it comes to excellent nursing care, there is no substitute for knowing the residents. Nursing teams are consistently assigned to each neighborhood so that individual needs and preferences are known. Teams include:
Certified Nursing Assistants are the caregivers closest to the residents and assist with activities of daily living, using individualized approaches to help residents achieve their goals.
Primary Care Nurses are licensed nurses who coordinate care during a specific shift, provide medication and treatments, and communicate care needs so that daily issues are addressed.
The Neighborhood Manager and Coordinator are registered nurses who are responsible to oversee the resident's entire plan of care over all shifts and facilitate an atmosphere where the resident is at the center of all decision-making.
Our Nursing teams, along with other professional staff members, work with your attending physician to develop an individualized plan of care. After initial admission visits, physicians see residents at Providence Extended Care routinely and coordinate all care through your Neighborhood Coordinator, Neighborhood Manager and Team Leader.
