Social Work Services

 

Providence Extended Care Center has four full-time social workers.  Upon your admission you will be assigned a social worker to assist in your discharge planning needs and other issues that may arise for you or your family during your stay. 

Discharge Planning
Most of those who come to Providence Extended Care Center are coming for specific care or treatment needs.  They want to return home or move to another setting when they no longer need skilled nursing or rehabilitation.  At Extended Care, we want to help our residents achieve this goal.  We begin planning for your discharge as soon as you are admitted, or sometimes even before.  Your progress is monitored regularly.  The RNs on your Neighborhood, your therapists and your social worker will all help you work toward discharge.  Your social worker will assist you in coordination in-home therapy or in arranging for special equipment needs at the time of discharge.
