My Checklist

 

1. Call Providence Admitting to Pre-Register for surgery and set up your Pre-op Clinic Appointment.

2. Check with insurance company to get pre-authorization if required.

3. Fill in the medical history work sheet to give to the Pre-Op Nurse.

4. If you have Diabetes, contact the Doctor managing your Diabetes for instructions.

5. Using the sheet include here, make an up-to-date list of all your medicines, vitamins, minerals and supplements. Bring that list with you to the hospital. If you do not have a list, bring your medicines with you.

6. Arrange for a responsible Adult to drive you home and stay with you after surgery. You may ride in a cab ONLY if you are accompanied by another responsible adult (NOT the cab driver).

7. Call your Surgeon's Office the day before your procedure to confirm the time of the procedure. Please be at the hospital one and one half hours before your surgery time, unless your doctor tells you otherwise. (Patients receiving an Epidural on the day of surgery need to arrive at least 2 hours before the scheduled surgery time).

8. Wear loose comfortable clothing; DO NOT WEAR ANY JEWELRY, MAKEUP, LOTIONS, FINGERNAIL OR TOENAIL POLISH. Any rings that are too snug to remove should be removed by a jeweler and left at home. Any hidden piercings or studs must also be removed.
