Screening, Education, Support and Exercise Services
Cardiovascular disease is a chronic, progressive condition. The good news is that you can control many of the factors that make it worse.
At the Center for Cardiovascular and Pulmonary Health and Rehabilitation, the goal of our screening, education, support and exercise services is to help reduce the impact of cardiovascular and pulmonary disease in our community. Our clinical staff help patients set reasonable goals, and design individualized programs to meet the personal needs of each person we serve.

Healthy Heart Exam
For individuals interested in preventing heart attack and stroke, the Healthy Heart Exam:

raises personal awareness of risk factors
-
motivates moderate to high-risk individuals to take action that lowers risk
-
detects high-risk individuals
-
prompts referral to doctors for follow-up.
The Healthy Heart Exam includes:

a resting ECG
-
a Heart Risk questionnaire
-
a cholesterol and blood sugar profile
-
blood pressure assessment
-
exercise habits assessment and recommendations
-
assessment of heart attack/stroke risk
-
consultation with staff.

Healthy Heart Program
If you’ve had heart attack, heart or vascular surgery, or angioplasty/stent, or are trying to prevent a heart attack/stroke, our Healthy Heart Program can help you reduce your chances of having a future heart attack and stroke through:
- education classes and materials
- one-on-one assessment of exercise tolerance
- an exercise prescription for our gym, at home or in your community
- follow-up risk factor assessments

Heartlink
Heartlink is a monthly educational and support group for patients and graduates of the Healthy Heart Exam, Cardiovascular Disease Rehabilitation and Risk Reduction programs. Please call 425-261-3780 for dates and times.
Pulmonary Rehabilitation
If you have lung disease, our Pulmonary Rehabilitation program can help you better understand and manage your disease, improve your tolerance to daily activities, decrease shortness of breath and fatigue, and counter anxiety or depression. This program offers you:
- education classes and materials
- one-on-one assessment of exercise tolerance
- exercise prescription for our gym, at home or in your community
- follow-up assessments

Lung Rangers
Lung Rangers is a monthly educational and support group for patients and graduates of the Pulmonary Rehabilitation program and other interested people. Please call 425-261-3780 for dates and times.
Stop Smoking Tool Shop
This class series provides you with the tools needed to quit smoking. You'll learn how to identify quitting barriers and develop strategies to overcome them. Please call 425-261-3780 for date, time and cost.
