To obtain copies of your medical records from PEMC (Colby campus, Pacific Campus, and Providence Pavilion), please see the procedure below:
1.) Download and fill out the Authorization for Release of Information form.
2.) Submit the completed form by either of the following methods:
a. Mail to: PEMC c/o ROI Department, PO Box 1147, Everett, WA 98206
b. Fax to: 425-317-0732
c. Or bring in the completed form to the Medical Records Department at the Broadway campus: 909 N. Broadway, Everett, WA 98201. Hours of operation are Monday through Friday 8:00am to 4:30pm.
Once your completed form is received it will be processed. Please allow 7 to 10 business days for the completion of your request. If you have any questions about the status of your request you can contact the Release of Information department directly at 425-317-0735.
Requesting Medical Records from Providence Physician Group
You can obtain copies of your medical records from Providence Physician Group by downloading and filling out the Authorization for Release of Information form, and turning it into your specific doctor’s office. Please allow 15 business days for your request to be processed. To follow up on the status of your request, you must contact your doctor’s office directly.
