Benefits effective July 1, 2011
Archdiocese plan members have two different medical plan options. Both plans feature preventive care and urgent care with no deductible. You can also choose your doctor and see a specialist without a referral on either plan.
New this year
There are important new benefits effective this plan year, some are as a result of Health Care Reform:
- Preventive care will be covered in full; you do not need to meet a deductible before the plan pays for certain preventive care services. View preventive services that are covered in full (PDF).
- Expanded coverage for adult children. Adult dependent children up to age 26 are eligible for coverage even if they are not a full-time student, no longer live with you, or are not dependents on your tax return or are married.
- The lifetime maximum will be eliminated. The lifetime dollar limit on essential health benefits is eliminated. Essential benefits include most major medical services such as doctor care, hospital care and prescription drugs.

For information about urgent care, travel outside the United States, definitions or other member services, visit our member website. You can also view benefit information unique to your plan by logging into myProvidence. Use your unique ID number found on your identification card to access secure online tools.
Are you a new Providence Health Plan member with the Archdiocese?
New members will receive an identification (ID) cards. Your card will have a unique ID number that will allow participating pharmacies and providers to access benefit information through secure online tools.
