Use this tool to plan for treatment costs. Enter the requested information below to estimate what you would owe (i.e. after insurance).

The information you need may be found by logging in to your My Providence account. There you will find:

  • your current deductible and out-of-pocket maximum balances and plan limits
  • if the deductible¹, out-of-pocket max¹, copay, and/or coinsurance applies to your treatment (found in your benefit summary)
As always, call Customer Service if you have any questions.

1. Results are NOT valid if:

  • the treatment cost entered is in excess of usual, customary, and reasonable charges (UCRs)
  • the service does not apply to the out-of-pocket max
  • the deductible applies to the out-of-pocket max
