Vision Riders
A vision benefit can be included as an endorsement to your medical plan. With this benefit, employees have access to either $200 or $300 in coverage during a two-calendar-year period. Employees do not need to meet any medical health plan deductibles, regardless of plan type, before accessing their vision care benefit.
Coverage can be applied to the following services:
- Vision examinations.
- Prescription lenses.
- Prescription contact lenses.
- Frames.
Using vision plan benefits
For the service to be a covered benefit, employees must receive all of their vision services and supplies from any licensed eye care provider. Vision examinations must be provided by an ophthalmologist or an optometrist. Employees submit their itemized receipts suitable for insurance billing purposes to the Providence Health Plan claims department for reimbursement.
