These maps illustrate where your company must be physically located and where a majority of your employees must work and reside to qualify Providence Health Plan group coverage. See your employer/group agreement for details.

Oregon Small Group

Oregon Small Group Service Area

Open Option: Employers must have at least 51% of their employees or members residing and working within the service area to be eligible for Open Option products and have access to our national network of providers.

Personal Option: Available to employers with all employees residing within Oregon or Washington. To be eligible for Personal Option products and have access to our national network of providers, employers must have 67% of their employees or members residing and working within the service area.
