Providence Health Plan members pay a $10 copay.
Providence Health Plan members may also choose to receive a flu shot from their participating provider and pay their office visit copay or coinsurance, or go to their local participating pharmacy and pay a $10 copay.
Providence Health Plan members must present their ID card at time of service.
The cost for employees who are not Providence Health Plan members is $28 from Fred Meyer and $25 from Safeway.
Services are available in Oregon and southwest Washington.
Employees who would like a flu shot should wear clothing that allows easy access to the upper part of the arm.
Flu Shot clinics vary in length. Approximate clinic times depend on how many employees participate:
- Up to 25 people – 30 minutes
- 25 to 50 people – 1 hour
- 51 to 75 people – 1.5 hours
- 76 to 100 people – 2 hours
