As a member of Providence Health Plan, you have access discounts on recreational and cultural activities and events in Oregon, Washington and Alaska. From health clubs, professional instructors, retail stores, to guide services, tour operators, performance venues, museums, theaters, massage therapists and much more.

Who is Eligible?

All Providence Health Plan covered members and their families. Participating locations will verify your eligibility when you present your Providence Health Plan member ID card.

  • Show your Providence Health Plan member identification (ID) card, along with a photo ID, at participating locations and identify yourself as a LifeBalance member.
