Registration Information
How do I create a myProvidence account?
You have a few options, depending on the type of myProvidence account you'd like to create:
- To create a Doctor's Office account, go to: http://www.providence.org/eHealth
- To create a Providence Health Plan member account, go to: http://www.providence.org/php/register.htm
- To create a general account, go to: https://www.providence.org/MyAccount/RegistrationViewer.aspx
Once on the myProvidence home page, you'll find links to services you can use to manage your health, your medical care and more.
At each of the above links, you'll see a registration form that asks you for the information we need to create that type of account for you.
After you've submitted your registration form, you'll be given instructions about any additional steps that need to be taken before you can access all the services you selected. For example, if you've signed up for myProvidence services that use personal health information, you will need to follow our "security activation code" process before gaining access to those services*. Please continue to the next question to learn about the myProvidence security activation code.
(*Note: Providence Health Plan members currently do not receive a security activation code.)
What is a "security activation code?" (Currently only for myProvidence Doctor's Office Providence Medical Group - Mercantile and Bethany clinics' pilot program)
When you sign up for myProvidence services that use personal health information, you'll be sent a security activation code that you must enter before gaining access to those services.
Your unique security activation code provides extra protection for the personal health information you share with myProvidence. When you complete the one-time process of entering your code, you confirm that you are the one person who should have clearance to view and manage your account. After that, we won't ask you for your code again. To help keep your personal information secure over time, log out when each time you finish using myProvidence.
The security activation code does not replace your myProvidence password. When you receive your code, you will need to log into myProvidence with your e-mail address and password. Then, a security activation code box will display for you to enter your code.
I manage the health care for everyone in my family. Can I use my e-mail address for all of their accounts?
No. For security reasons, myProvidence now requires a unique e-mail address for each myProvidence account.
If you have a myProvidence account that had been sharing an e-mail address with another account, you will be required to provide a unique e-mail address the next time you log in. All of the information in your account will be preserved.
We recognize this requirement may be difficult for parents who oversee their family's health care, and are working to make it easier to manage multiple accounts in the future.
Providence Health Plan member services
How do I access my health plan claims information on the myProvidence site?
Navigate to the Providence Health Plans Web site. If you haven't yet registered for myProvidence, go to the Members section and select "Register".
You will need to provide your member identification number and your group number to complete registration for Providence Health Plans services. These can be found on your member ID card.
If you've previously registered for general myProvidence account and would like to add Providence Health Plans services to your account, follow these steps:
- Log in and click on the "Providence Health Plans" tab at top.
- Follow the "Sign up for Providence Health Plans access" link to add these services to your account.
I am a Medicare, Medicaid or OHP Member — Can I register on your Web site?
Currently, Medicare, Medicaid or OHP members cannot access personal health plan account information online. However, you can create a general user myProvidence account.
Dependents / Spouse Information
Can my covered spouse and dependents view their claims and personal information online?
Yes. Accounts are set up on an individual basis for privacy and security protection. Your covered spouse, or child age 15 or older, can register online to access their own health plan account information.
What information can my covered spouse or dependent (age 15 and older) view on the Web site?
Once your spouse or dependent has completed their myProvidence registration, he/she can view their medical claims online, view their summary of benefits and handbook and order a replacement member identification (ID) card. Oregon PEBB members also can select their medical home clinic and doctor, and can complete their health risk assessment (HRA). Your spouse or dependent cannot, however, view other family member's health plan account information.
Help / Troubleshooting
I forgot my password. What should I do?
Look for the “Forgot your password?” link on the myProvidence login page. Enter the e-mail address you provided when you registered for myProvidence, and we'll automatically send instructions for resetting your password to the e-mail address that was used to create the account.
How do I change my e-mail address, password, address, phone number, or account preferences?
Follow these steps:
- Log in to myProvidence with your e-mail address and password.
- Click on the "My Account" link at top.
- On the "Update personal information" link and submit the changes to your account.
Please note: myProvidence uses the e-mail address you provide for critical communications related to your account, including notifications when you have unread mail in your message center inbox.
*Notes to Providence Health Plan members:
- Changes to your myProvidence account will not be reflected in your personal information on file with Providence Health Plan (PHP). If you have employer group benefits please update your information with your benefits administrator.
- If you are an Individual and Family Plan member, contact Customer Service to update your information.
- In order for you to access myProvidence health plans services the following information must match what is shown on your Providence Health Plans member ID card: your name, birth date, member number and group number.
I created a health plan account, but I can't access it or I'm receiving a message that says there is a problem with my account.
Reasons why your account may not be working:
- Your personal information or your plan benefits have changed.
If your name or e-mail address has changed, or if you have changed employers, you will need to update your myProvidence account information.
If you are no longer a Providence Health Plan member, your myProvidence account will change to a general user account. You will still have access to myProvidence services but you will no longer have access to Providence Health Plans online services.
Solution: Log in to myProvidence and follow the directions on the page. You can update your account information from the My Account link located at the top of your screen.
- You attempted to create a duplicate myProvidence account.
The myProvidence system only allows a member to create one myProvidence account per member number. If you have previously signed up for a myProvidence health plan account, your account is already set up. The system does not allow duplicate accounts to be created.
Solution: Log in with the e-mail address and password that you created when you registered for your myProvidence account. If you forgot your password, go to the Log in page and click on the "Forgot your password?" link.
- At one time, you had several myProvidence accounts tied to the same e-mail address.
If you have a myProvidence account that had been sharing an e-mail address with another account, you will be required to provide a unique e-mail address the next time you log in. All of the information in your account will be preserved. If you experience problems see below.
Solution: Use our online message box to let Customer Service help you reset your account.

Health Information and the Providence Mission
Why do you publish information about medical services that are not consistent with Catholic teaching?
In our effort to be faithful to the Catholic faith tradition and identity of Providence, we try to provide information about medical services that are consistent with our moral tradition. At the same time, we believe — as studies have suggested — that giving people easy access to reliable health information on the Internet makes for better informed and healthier patients. We partner with a select group of highly-regarded health content providers to provide you with trustworthy medical information on our Web site. When choosing these partners, we carefully consider whether their commitment to health care is consistent with the Catholic Church's teachings. We also explore whether their standard presentation of content can be customized to reflect our faith tradition. Our goal is to help you manage your health while staying true to our Catholic identity and to our Mission and core values. We appreciate your patience and understanding as we continually refine our efforts to do that.
If you have additional comments or questions about what you find on our site, we invite you to send them to phpmission@providence.org.
