About Provider Listings

The searchable provider directory contains participating provider listings for members of Providence Personal Option and Open Option (EPO) plans and for Providence Health & Services Alaska, Providence Health & Services Oregon, Providence Health & Services Washington and Peace Health employee self funded plans administered by Providence Health Plan, Providence Health Plan's Oregon Health Plan (OHP), Providence Medicare Plans, Providence Traditional Option Plus (TO+) plans, Providence Choice Plan (PEBB), Providence Individual and Family Plans, Oregon Educators Benefit Board (OEBB) and providers affiliated with Providence Health & Services (PHS) hospitals.
Also contained in the directory are provider listings in the Providence Preferred (PPO) network, the Providence MCO network and facility locations of pharmacies, clinics and hospitals.
Not all physicians or providers affiliated with Providence Health & Services are contracted with Providence Health Plan or Providence Preferred. Provider listings and our searchable provider directory are for reference only. Plans vary based on what riders your employer has purchased for you.
