Premium Assistance Through FHIAP
At Providence Health Plan, we are committed to providing quality health care at an affordable price. We also recognize that for many uninsured Oregonians, paying for health care coverage is still a challenge. That’s why we are pleased to announce that premium assistance is now available for three Providence Individual and Family plan options through the Family Health Insurance Assistance Program (FHIAP).
About FHIAP
FHIAP is a program that helps uninsured individuals and families in Oregon buy health insurance. For those who meet eligibility requirements, FHIAP provides subsidies (grants of money) that may pay up to 95 percent of your monthly premium.
The following Providence Individual and Family Plans have been approved for FHIAP assistance. Applications may be submitted for the following plans:
- Optimum 500
- Optimum 1000
- Value 500
Need Premium Assistance?
Apply with FHIAP first. You must apply with FHIAP before you sign up for health insurance coverage. FHIAP will let you know within 30 days whether you qualify for a subsidy and provide you with further instructions. To apply for a subsidy, call FHIAP at 1-888-564-9669 or visit www.fhiap.oregon.gov
