Individual Coverage

Providence Health Plan has recently updated its enrollment policy for applicants and dependents under age 19. Read more here.

To apply for individual plan coverage, you must:

  • be 18-64 years of age
  • not eligible for Medicare and
  • live in our Service Area (PDF)

Apply for individual coverage. You will be on an individual plan. This means you will be the only person covered on the plan and you will be "the plan policyholder." Your monthly premium is determined by your age and the type of plan you choose.

Family Coverage

A family plan can include:

  • you and your spouse ages 18-must live in our Service Area (PDF); refer to the rate for Individual & Spouse
  • you and your dependent children ages 0-26; refer to the rate for Individual & Children
  • you, your spouse and your dependent children; refer to the rate for Individual & Family

Apply for family plan coverage. The premium is determined by the oldest family member applying for coverage.

Child-Only Coverage

To apply for dependent-only coverage, your child must:

  • be newborn to 17 years of age
  • live in our Service Area (PDF)

Coverage just for children: You can apply as the plan policyholder for your dependent child's coverage. You must apply for a separate policy for each person age 0-17. Refer to the rate for Individual, age 0-17.
