Dedicated to improving your health and quality of life, a registered nurse will help you take an active role in your chronic medical conditions or complex health care needs. Members receive condition-specific information, education and support. Employers can be confident that employee health conditions are receiving the attention they require.
We provide support for the following conditions:

How we support you:
- Educational pieces mailed to your home
- Personal, private telephone consultations with a registered nurse
- Answers to your health questions by phone or email
- Access to health and wellness classes in Alaska, California, Oregon and Washington
To participate in our Care Management Programs:
- You can enroll yourself
- Your doctor may recommend you
- You may receive a call from us if you’ve been seen by a physician for a chronic medical condition or been hospitalized
Unsure if you are at risk for a chronic condition?

Not available to Medicare, Oregon Health Plan and some ASO members.
Your results will not change your Providence Health Plan eligibility and you cannot be disenrolled from the plan due to your answers on the HRA.
