Health Education Reimbursement

The following information applies to health education reimbursement outside of the Portland area. For discount information within the Portland area, please see the detailed information listed for each class in the Providence Health and Fitness online directory.

Guidelines for Reimbursement

Providence Health Plans (PHP) wants you to be healthy and to live well. That's why we offer member discounts or reimbursements for health education classes. To qualify for reimbursement, the following must apply:

  1. All classes must be sponsored by a hospital that contracts with PHP. This includes hospitals and facilities that are part of our national network available to members. Search our Provider Directory and search for Facilities.
  2. Class payments must be paid in full by the PHP member. Health education classes offered at a discounted rate through a PHS facility are discounted and are not eligible for reimbursement.
  3. The PHP Health Education Reimbursement form (PDF 61kb) must be completed. See the Getting reimbursed section below for more information.
  4. PHP members must have valid and active member status at the time of class registration to be eligible for reimbursement.

PHP Core Health Education Classes

Childbirth Education

  • Weekly series
    PHP will reimburse after the first $25
    Up to an $85 class fee
    ($60 reimbursement max.)
  • Weekend seminar
    PHP will reimburse after the first $55
    Up to a $115 class fee
    ($60 reimbursement max.)

Any amount over is the responsibility of the member. Other classes related to infant care, feeding, etc., are not eligible for reimbursement, including childbirth refresher classes.

Weight Management

PHP will reimburse after the first $55
Up to a $180 class fee
($125 reimbursement max.)

Any amount over $180, or all fees if the course is not completed, is the responsibility of the member.

Smoking Cessation

PHP will reimburse after the first $15
Up to a $175 class fee
($160 reimbursement max.)
Any amount over a $175 class fee, or all fees if the course is not completed, is the responsibility of the member.

Classes must be sponsored by a Providence Health Plan-contracting facility (see our Provider Directory).

Getting Reimbursed

If you participate in one of these health education classes at one of the facilities listed here other than a Providence Health & Services facility, you may complete a PHP Health Education Reimbursement form (PDF 61kb). Please complete all the information requested and mail it to the address provided on the form.

For PHP members who live in the Portland-metropolitan area, PHP contracts with Providence Health & Services Health Education Services for access to discounted classes. View our complete list of discounted classes available through Providence Health & Services in the Portland area. PHP will not reimburse Portland-metropolitan area members who pay for health education classes from other sources, including the medical offices of contracting physicians. Reimbursement for health education classes is only available at non-Providence Health & Services facilities outside the Portland area.

NOTE: Class availability and PHP member cost-sharing may vary based on location and employer group contract terms.
