Your Member Identification Card
Your Providence Health Plan (PHP) member identification (ID) card shows your name, group number, member number and basic copay/coinsurance information. On the back you'll find Customer Service information and Providence RN, our free 24-hour nurse medical advice line.
You may order a replacement member identification card online after registering to access your health plan online services, or by calling your Customer Service Team.
Take your member ID card with you to every medical appointment to verify your copayment, coinsurance or deductible amounts, if they apply.
Before You See a Physician or Other Provider
- View our Provider Directory for a list of participating personal physicians/providers, specialists, pharmacies, and other facilities. To request a paper copy of the directory, contact Customer Service.
- Call the physician/provider office to see to make sure they are accepting new patients and verify any copayment or coinsurance due at the time of your appointment.
- For out-of-area care or to see a non-participating providers, check your Summary of Benefits for benefit details.
Ordering a New Member Identification (ID) Card
Q. I am a new member — do I need to order an ID card?
A. We will automatically send a member identification card to new members in the mail. You should receive your new member ID card around the same time that your new member packet materials are received.
Q. How do I order a replacement member ID card?
A. Log in to your myProvidence account* and select the link that says "View my health plan account information online". Select the "Order ID Cards" link and follow the instructions. You will receive your new member ID card within 5 to 10 business days. You can also contact Customer Service to order a new card.
Q. My spouse/son/daughter needs to order a new member ID Card. How can they do this?
A. Your spouse, or dependent child over the age of 15 will need to registering online to set up their own myProvidence health plan account. They can then log in to order a new ID card, view their own benefit plan materials and medical claims information.
*Note: The health plan online account features are currently available to members of our Providence Personal Option, Providence Open Option, PEBB and Providence Traditional Option plans only.
