Text Size: Your mobile member ID card
Get your account information on the go! Providence Health Plan members can view, email or fax an image of their member ID card to their provider. Our smartphone app for iPhone, Android and BlackBerry will let you:
- View your ID card
- View all cards on your policy (spouse and/or dependents)
- Email or fax ID card image to your doctor, hospital or pharmacy
You can download the app by searching "Providence e-card" or "Providence Health Plan" in your smartphone's app store or directly from Android Market, iTunes or BlackBerry App World.
Getting access
A four-digit PIN and your member identification number is required to access this app. All member ID cards printed after June 21, 2011 will include PIN information in your mailed packet. PINs will be located on the back side of the ID card.
If you do not have an ID card with a 4-digit PIN printed on the back side of the card, you may call Customer Service at 503-574-7500 or 800-878-4445, Monday through Friday, 8 a.m. to 5 p.m. or request by secure email at http://www.providence.org/php/contactus (make sure to provide your member identification number for verification). Medicare members: Please call 503-574-8000 or 800-603-2340, Monday through Friday, 8 a.m. to 8 p.m. to request your 4-digit PIN.
