How do I find a doctor?

You can look for a doctor by visiting our online provider directory. You will be able to look for primary care providers, specialists, hospitals and urgent care facilities. You find a provider close to your home by using the zip code search. If you have questions or need help finding a provider call the Providence Oregon Option Customer Service Team at 1-800-898-8174 or 503-574-8200 or for the hearing impaired 1-888-244-6642. Our office hours are Monday through Friday, 8 a.m. to 5 p.m.

What if I want to change my Primary Care Provider (PCP)?

If you would like to change who your Primary Care call the Providence Oregon Option Customer Service Team at 1-800-898-8174 or 503-574-8200 or for the hearing impaired 1-888-244-6642. Our office hours are Monday through Friday, 8 a.m. to 5 p.m. We will also be able to help you find a new doctor if you are unsure where to go.

What pharmacies can I go to fill my prescription?

What is urgent care?

Many situations are not emergencies, but do need immediate medical attention. Urgent care is treatment you need right away for an illness or injury that is not life threatening, but you cannot wait for a doctor's office appointment.

Some conditions for which you should use urgent care include:

  • minor cuts or burns
  • ear, nose and throat infections
  • sprains or strains
  • stomachaches
  • headaches or dizziness

Many Urgent or Immediate care clinics open early, close late and have weekend hours. If you are not sure what to do, call your doctor or Providence RN for advice at 503-574-6595. They are available 24-hours a day and can direct you to the most appropriate place for care.

What is emergency care?

An emergency is an accident or sudden unexpected illness that needs to be treated right away or it could result in loss of life, serious medical complications or permanent disability.

Examples of emergency medical conditions include:

  • heart attack or severe chest pain
  • loss of consciousness
  • bleeding that does not stop
  • severe abdominal pain
  • stroke, sudden paralysis or slurred speech
  • medically necessary detoxification

In an emergency, always seek medical care immediately. Go directly to the nearest emergency facility or call 9-1-1. You and your covered family members can receive emergency medical services anywhere, 24 hours a day, even when you're away from home.

What information will I receive when I enroll?

  • DMAP coverage letter
  • Providence Health Assurance member ID card
  • The Providence Oregon Option Primary Care Provider (PCP) Directory. A PCP Directory is included with your member packet. It lists Providence Oregon Option participating primary care physicians and providers (PCPs), women’s health care providers, hospitals and urgent care centers, vision providers and pharmacies.
  • The Client Handbook for the Oregon Health Plan. The State of Oregon sends you this book. Your Client Handbook gives you important information about:
    • Covered and non-covered medical services
    • Required copayments (if applicable)
    • Dental plan information
    • Mental health plan information
    • Your rights and responsibilities as a member
    • Solving problems and the complaint process
