Providence Health Plans Options

Thank you for choosing Providence Health Plan, with in-plan access to most doctors in Oregon and local, fast and friendly customer service.

There are three Providence Health Plan options offered for Oct. 1, 2009 – Sept. 30, 2010.

All of our OEBB options feature low copays with no deductible for routine exams, wellness services and urgent care when using in-plan providers. You also can choose your doctor and see a specialist without a referral on our plans. View the information below for more details.

Pharmacy Benefits

Our prescription benefits have no deductible. We have affordable generic options and an extensive pharmacy network, including across the nation.

For your convenience, 90-day supplies of prescriptions are available from mail-order and local retailers such as Costco, Walgreens, Safeway, Fred Meyer, QFC and Yoke's.

Access to Physicians and Hospitals

Providence Health Plan provides in-plan access to more than 10,000 participating providers in Oregon and southwest Washington, and access to an extensive national network of providers across 50 states.

The ability to choose a doctor and see a specialist without a referral.
Chiropractic, acupuncture and naturopathic healer access in-network through the ASH network.
Out-of-plan benefits apply when you see any nonparticipating provider. Alternative care is not available out-of-plan.

Our plans are designed to keep members well. All in-network preventive services such as periodic health exams are fully covered.

Sign up now for our e-newsletter, Wellness Watch. Newsletter subscribers receive health and wellness information monthly. We are careful with your personal information and won't give your e-mail address to anyone else.

Health and Wellness Included

Access to online wellness information including a personalized health report and motivational action plan are available via secure online tools from our Health Balance^SM program.
If you are a current member, 18 years or older, log in to myProvidence, click on the health plan tab and follow the link to "Assess your health risks." You will need to be a current member of Providence Health Plan and register with myProvidence before you have access to the tools and information.
Online access to diagnoses, medical cost estimates and bills are available through myProvidence.

Urgent Care, Nurse Hotline, and Emergency Room Visits

For medical questions or to be seen quickly, urgent care is available at a low $25 copay.
Free medical advice from Providence nurses available by phone, 24 hours a day, seven days a week, through ProvRN.
Emergency room visits for an emergency medical condition are a $100 copay, waived if admitted within 24 hours.

If you are transitioning to a Providence health plan, it is very important for us to understand any special health needs or serious medical conditions that you or your family members may have.

Transition of care benefits may apply if you or a family member are currently being treated for a serious medical condition such as cancer treatment. Review and complete the attached form if you are currently undergoing medical treatment and would like to apply for care transition.
