Online health and wellness tools

Participate in a free online health risk assessment and receive a motivational action plan and resources by mail to prevent or manage disease. Online tools and health information are accessible when you login or register with myProvidence.

Tobacco cessation

  • $0 copay coverage for over-the-counter nicotine gum and patches
  • $0 copay coverage of buproprion, a drug that blocks nicotine dependency in the brain
  • $5 copay (full-time employees)/$10 copay (part-time and retirees) for Chantix, a drug that reduces cravings for cigarettes and other tobacco products

Value Plus Pharmacy

$0 copay on commonly prescribed generic and brand-name drugs to treat conditions such as asthma, chronic obstructive pulmonary disease, congestive heart failure, coronary artery disease, depression and diabetes. See the complete list of Value Plus prescription drugs (PDF).

  • $0 copay colorectal cancer screening
  • $0 copay breast and cervical cancer screenings
