Sign up for secure electronic fund transfers for commission checks and have your money deposited directly into your bank account each month.

Print the direct deposit form (PDF), fill it out, sign and send it via secure fax to Providence Health Plan at 503-574-8150.
When will direct deposit take effect?
- It may take one to three months to process the form.
- Commission checks will be mailed until direct deposit setup is complete.
- Deposits will occur on the 15th of every month or next business day.
- Monthly commission statements will continue to be mailed.
