Individual and Family Plans emphasize wellness with low copays
Low copayments for preventive care services and personal physician office visits are featured in all 12 individual and family plans, effective Nov. 1, 2008.
Now available for online quoting, these plans also include vision and prescription benefits, access to ProvRN, our 24-hour Providence nurse advice line, and discounts on gym memberships and other healthy activities through www.lifebalanceprogram.com.

Other benefit changes
- Vision care: Previously, vision care was excluded. Beginning this contract year, routine vision care (including exams, lenses, frames and/or contacts) is a covered benefit.
- Urgent care services (Optimum and Value plans): Previously, urgent care services were subject to deductible. Beginning this contract year, the deductible is waived for urgent care services.
- Durable medical equipment: Previously, benefits were limited to $2,500 per member per year. Beginning this contract year, the $2,500 limitation has been removed.
- Emergency services (Optimum and Value plans): Previously, coverage included a $125 copay with deductible waived. Beginning this contract year, members will pay a $125 copay that is subject to deductible.
- Chiropractic services: Previously, coverage included 15 chiropractic visits per calendar year. Beginning this contract year, coverage for chiropractic services will be excluded.
