As a Providence Health Plan member, you have access to discounts on eye exams and eyeglasses through Visionworks locations in Oregon and southwest Washington. This is not a covered benefit under your medical health plan, but is a member extra value service.
Who is Eligible?
All Providence Health Plan covered members and their qualified dependents, unless otherwise noted. Some restrictions apply. The participating provider will verify your eligibility when you present your Providence Health Plan member ID card.
Oregon Health Plan members enrolled with Oregon Option Standard are eligible for vision discounts through Visionworks. Discounts are not available for Oregon Option Plus members.
Eye Exam & Eyeglass Discounts
- Vision examination*: Save 20 percent
- Eyeglass frames*: Discount based on frame type
- Eyeglass lens*: Single vision $59.99; Bifocal $82.49, Trifocal $119.99, Progressive $119.99
- Eyeglass lens upgrade: Based on lens type, $20 per upgrade including; UV Coating, Edge Polish, EG protection and Tints. A 25 percent discount is available on AR coating.
