About ProvLink
ProvLink is a password protected site for physicians and clinic staff contracting with Providence Health Plans.
Features include eligibility, referrals, claims look-up, plan option information, manuals, directories, policies, news, newsletters, preventive guidelines, clinical and disease management tools, credentialing information, forms, prior authorization lists and other detailed resources.
To obtain a ProvLink username and password, please fill out the ProvLink User Form (PDF 31kb) and Confidentiality Statement (PDF 66kb) and fax to the number listed on the ProvLink User form. Your account information will be mailed within two weeks.
Providence Health & Services, PMG Logician, electronic medical textbooks and journals and PHS e-mail are also available to our physicians. For information about setting up a Providence Hospital System account for any of these services, please contact Physician Information Services at 503-216-8060.
