To obtain a ProvLink username and password fill out:
- ProvLink User Form (PDF 31kb)
- Confidentiality Statement (PDF 66kb)
Fax to the number listed on the ProvLink User form. Your account information will be mailed within two weeks.
Providence Health & Services, PMG Logician, electronic medical textbooks and journals and PHS e-mail are also available to our physicians. For information about setting up a Providence Hospital System account for any of these services, please contact Physician Information Services at 503-216-8060.
