Transitional Care Unit
Patients of all ages are admitted to the Transitional Care Unit at Providence Mother Joseph Care Center. This unit is one of the largest in the Southwest Washington area—admitting and discharging over 700 patients per year.

The focus of this unit is to help patients make the transition from hospital to home following an accident, injury, surgery, hospitalization or a medical condition, and end of life care. The unit specializes in:

  • Rehabilitation  
  • Wound Care  
  • I.V. Therapy  
  • After Surgery Care  
  • Cardiac Care  
  • Trauma Care

The Transitional Care Unit offers the latest in medical practice and technology, combined with an exceptional standard of caring that is set by all staff.

We design a plan of care in order to return the patient home or to a lesser care environment as soon as possible. We do this by utilizing:

  • Case Management
