For example, according to the American Heart Association (AHA), ten percent of all hospital admissions are the result of patient failure to take prescription medications In fact, AHA statistics show more than half of all Americans with chronic diseases don't follow their physician's medication and lifestyle guidance.
The CVDRP offers a new strategy – at no cost to your patient or your practice – Clinical Care Coordinators can aid compliance, optimize treatment and improve outcomes by helping your patients understand and follow through on treatments and testing. We also facilitate interdisciplinary communication, help centralize patient data and provide you with regular progress reports. Our future goals include a dedicated ambulance service and facilitated home monitoring.
