The primary goals of health care are to prevent and cure disease, Hospice, however, can make a real difference when treatment options become less optimal. As a patient and family approach end of life care, it is difficult to talk about. If it is time to emphasize comfort over cure patients, families and their physicians can turn to Hospice for skilled and compassionate care. While providing for the patient's medical needs, hospice staff manage symptoms. The focus of care is to maximize the quality of life for both patient and family. The choice for Hospice is not an easy one, but can be the right one. Many of our families tell us they wished Hospice had been chosen earlier.

TrinityCare Hospice, owned by Providence Health System, was established in 1977 and is the oldest and largest non-profit hospice in Southern California. Medicare certified, licensed and accredited, our Hospice is a nationally recognized leader in innovative and quality care. Services are offered throughout the Los Angeles area.

 

By providing medical expertise, nurse management, emotional and spiritual support, our interdisciplinary team of professionals and volunteers help the patient and family to develop a plan of care that is satisfactory to them. Affirming the quality of daily life for all, Hospice focuses on expert pain and symptom control so that those with terminal illness can live an alert and pain free life. Care is most often provided in the patient's home, but can be offered in a contracted Skilled Nursing Home, Assisted Living or Board and Care environment

Services Provided:

Hospice offers individualized and coordinated services to promote optimum comfort and a personal sense of control, which includes:

  • A plan of care based on the patient's needs and goals
  • Expert pain and symptom management
  • Physical, emotional and spiritual care for the patient and family
  • An interdisciplinary team of professionals and volunteers
  • Bereavement and grief support for the children and adults
  • Respite and attendant care
  • 24-hour/7 day a week availability
  • Medications, equipment and supplies delivered to the home

 

The Hospice Team includes:

  • Attending Physician and Hospice Medical Director
  • Registered Nurses
  • Clinical Social Workers
  • Chaplain
  • Registered Dietitian
  • Home Health Aides
  • Trained Volunteers
  • Physical, Occupational and Speech Therapies
  • Liaison Nurses                                                                              

Information you might consider:

  • A physician order is needed for Hospice care.
  • Attending physicians continue to provide care. The Hospice interdisciplinary
    team supplements physician care. The physician continues to bill for physician visits.
  • Hospice care is appropriate for many end stage terminal diagnoses, including cancer, heart, lung, liver and kidney disease, ALS, Alzheimer's/Dementia, and AIDS.
  • Eligibility for Hospice care is based on a physician's certification that a patient's prognosis is 6 months or less if the illness follows its normal course. Hospice services are not time limited. Should a patient stabilize, care may be discontinued until it's needed again.
  • Reimbursement is available through the Hospice Medicare and Medical Benefits, contracted Managed Care Systems and Private Insurance. Equipment, medications and supplies related to the primary illness are included. A patient's insurance for all care unrelated to the terminal illness remains in place. No one is declined service because of inability to pay; underinsured and uninsured patients are supported by the TrinityCare Hospice Foundation.
  • Hospice provides intermittent team member visits on a regular basis and 24 hour telephone access to nursing staff for information, support and visits if needed. The intensity and frequency of nursing care can be increased depending on the patient's medical needs.
  • People who live alone are eligible for Hospice as long as they are safe, but need to be willing to choose in-home assistance or placement when they can no longer manage.
