Clare Cross joined Providence Health & Services in the fall of 2005 as the nurse coordinator for the Providence ALS Center. Some of her roles include screening patients prior to clinic to identify needs, educating patients about the disease process, therapy, and treatment, arranging for referrals to specialists, and most importantly, acting as the liaison between team members and patients to ease communication and assure patients needs are being met in a timely manner.
Prior to joining Providence, Clare worked on cardiac units at OHSU and Emanuel for 15 years and in an outpatient clinic as a triage nurse with internal medicine, pediatrics, and OB/Gyn for three years.

Dr. Louis Libby graduated from Tufts University with a B.S. in Psychology and an M.D. with distinction from George Washington University School of Medicine. His internship, residency, chief residency, and fellowship training in pulmonary and critical care medicine were completed with the University of Colorado Health Sciences Center, Denver.
Dr. Libby is board certified by the American Board of Internal Medicine, Subspecialty Board in Pulmonary Diseases, and Subspecialty Board in Critical Care Medicine. Professional memberships include the American Thoracic Society, the Oregon Thoracic Society, the American Lung Association of Oregon and the American Sleep Disorders Association. Dr. Libby is also a fellow with the American College of Chest Physicians.

Dr. Thomas Schaumberg received his B.S. in molecular, cellular and developmental biology from the University of Colorado, Boulder and his M.D. from the University of Colorado Health Sciences Center, Denver. His internship and residency training were completed at Oregon Health & Science University. Postdoctoral Fellowship training included pulmonary medicine, critical care medicine, and a post-doctoral research fellowship in cardiovascular research at the University of California, San Francisco.
Dr. Schaumberg is board-certified with the American Boards of Internal Medicine, Pulmonary Medicine, and Critical Care Medicine.

Dr. Lefor received his B.S. in biology from the University of Portland, and his M.D. from Oregon Health & Science University. He completed his internship and residency at Virginia Mason Medical Center in Seattle and his pulmonary and critical care fellowship at the University of Utah Medical Center in Salt Lake City.
Dr. Lefor is board-certified with the American Boards of Internal Medicine, Pulmonary Medicine and Critical Care Medicine.

Dr. Sax has had extensive training in treating nerve and muscle conditions, with an emphasis on diagnosing and treating neuromuscular complications in pregnancy and inflammatory neuropathies. She earned her medical degree from Oregon Health & Science University and completed residencies at Providence Portland Medical Center and Harvard Longwood Neurology Training Program. Dr. Sax also attended Harvard Medical School for a fellowship in neuromuscular study.

Dr. Phillips attended college at Oregon State University and received his Bachelor of Science. He then attended medical school at Oregon Health & Science University in Portland, OR, where he received his medical doctorate. For his internship, residency, and fellowship training, Dr. Phillips attended the University of Colorado Health Sciences Center in Denver. He is board-certified in both internal medicine and gastroenterology. Professional memberships include The North Pacific Society of Internal Medicine, the American Gastroenterology Association, American Society for Gastrointestinal Endoscopy, American College of Physicians, and the American Society of Parenteral and Enteral Nutrition.
Currently, Dr. Phillips serves as the medical director of the Metabolic Support Team and is chairman of the Medical Education Committee at Providence Portland Medical Center. He is interested in overseas medicine and spent 6 weeks at Moi University, Eldoret, Kenya as an attending physician in internal medicine and gastroenterology in 2002. His area of specialty in gastroenterology includes endoscopic ultrasound.

Lance Christian is the services director of the ALS Association of Oregon & SW Washington. He also serves as the Providence ALS Center Social Worker. Lance received his B.A. in history from Reed College, and his M.A. in social work from Portland State University, both of Portland, OR. He has worked in community mental health for many years but most recently has focused on neurological health care working at the ALS Association, The National MS Society and the MS Society of Victoria, Australia.
Lance’s area of focus includes supporting families to access the resources of the ALS Association and other community resources, insurance advocacy, support groups and family care giving supports.
Debi Corah, R.R.T. Respiratory Therapist, Providence ALS Clinc
Kathy Cutter is a physical therapist who received her master's degree in physical therapy in 1998 through the UCSF/SFSU Joint Graduate Program in Physical Therapy in San Francisco. She has worked at Providence Portland Medical Center since 1999, is a current member of the outpatient neuro rehabilitation team as well as treating orthopedic and aquatic physical therapy clients. She has been a lead investigator in clinical research relating to balance and fall risk in persons with osteoporosis.
Kathy joined the ALS team in 2005. Her special interests and continuing education have focused on vestibular therapy, balance reeducation, wheelchair seating, and rehabilitation for those affected by stroke, spinal cord injury and other neurological conditions.
Debbie Fulkerson is an occupational therapist who received her B.S. in occupational therapy from the University of Kansas in Lawarance, KS. She has worked in the area of physical medicine and rehabilitation with primarily neurological conditions. She has worked in outpatient and inpatient facilities including V.A. Hospital in Marinez, CA; John Muir Medical Center in Walnut Creek, CA; UpReach Pavillion Rehabilitation Hospital in Gainesville, FL and Vencor Rehabilitation Services in Gainesville, FL. She transferred to the Salem, OR area with Vencor in 1998. She has worked for Providence Portland Medical Center inpatient and outpatient settings since 2000, and joined the ALS team in 2005. Debbie's special interests include advanced training and completion of certification course work for neurodevelopmental therapy approaches for adult movment disorders following brain injury from strokes, traumatic brain injury; tumors; and other neuroligical diseases.
Robert Love, O.T. Occupational Therapist, Providence Outpatient Rehab
Karen Smith is a speech-language pathologist who received her B.A. in communicative disorders at University of Wisconsin-Madison and her M.A. in communicative disorders at University of Wisconsin-Eau Claire. She worked at the Head Injury Rehabilitation Center in Juneau, WI before moving to Portland in 2001. She has worked for Providence Portland Medical Center in adult outpatient neuro-rehabilitation since her move to Portland. She joined the ALS team in 2005. Karen's special interests include augmentative & alternative communication, dysphagia, and cognitive-communication disorders following strokes, head injuries and other neurological conditions.
