Social Work Services

 

Changes in patients' health status pertaining to home safety, psychological or cognitive issues, patient and/or caregiver stress, or additional and increasing care needs signal the need for social work assistance. Providence social workers can provide:

  • Resources for hiring in-home assistance
  • Referrals to aging services and public assistance when needed
  • Help with community resources
  • Assistance with financial and legal concerns
  • Advice on probate issues, such as wills or trusts
  • Medicaid/Medicare and supplemental insurance information

Our social work team:

  • Offer emotionally supportive counseling
  • Provide conflict resolution
  • Teach positive coping skills and mechanisms (i.e. relaxation techniques and goal setting)
  • Engage in family/team interventions
  • Network with a host of community resources and support groups
  • Enlist help of other Home Health specialists as part of the care team when needed
  • Provide resources for additional in-home assistance, placement options and financial concern
