Resources for Surgery Patients

Get the best possible care

To make sure you get the best, safest care possible, we need some important information about you. Before you come to the hospital for surgery, please complete and bring with you a copy of this medication history form and the health summary form. If the patient is a child aged 13 years or younger, the parent or guardian should complete the pediatric CAD instead of the health summary. Your safety and your health are important to us.

Instructions and what to expect

Walk through our Step-by-Step Guide for Surgery Patients to learn what to do and what to expect during all phases of your surgical experience at Providence. Learn more about the procedure you will have performed by reading in "Medical Reference Library" in Medline Plus.

Need a place to stay?
