To make sure you get the best, safest care possible, we need some important information about you. Before you come to the hospital for surgery, please complete and bring with you a copy of this medication history form and the health summary form. If the patient is a child aged 13 years or younger, the parent or guardian should complete the pediatric CAD instead of the health summary. Your safety and your health are important to us.
Free shuttle service offered
Patients, visitors and employees of Providence St. Vincent Medical Center can enjoy Providence's free shuttle service for those commuting via TriMet bus and MAX. Our wheelchair accessible vans run approximately every 20 minutes between the Sunset Transit Center (10470 SW Barnes Rd.) and hospital, from 5:30 a.m. to 10 p.m., Monday through Friday.
