We are happy to provide estimates of the charges that will be incurred in providing your care. However, the portion of these charges that you will be responsible for paying (your out-of-pocket costs) depends on your insurance coverage. Some insurance companies cover a set percentage of our actual charges, while others cover services based on a pre-determined rate. To understand what your insurance covers and what portion of the charges you will be responsible for paying, please contact your insurance company.
Many of these questions are answered on your insurance card. This sample insurance card can help you read and understand the coverage listed on your card.

To determine how much you will owe, review the Medicare and You 2008 Handbook, published by the Center for Medicare Services. Pages 110–113 of the Handbook list deductible, coinsurance and premium rates for 2008.
Some services may not be covered by Medicare. If this is the case, you will be asked to sign an Advance Beneficiary Notice (ABN) and you will be financially responsible for the charges for that service. Page 85 in the Medicare Handbook describes ABNs.
