Providence Benedictine's Home Health team serves patients who qualify for receiving skilled nursing care or therapy in their home. These professionals also assist homebound chronically ill patients and their families by offering help, support and medical guidance. Our team works in partnership with the patient's physician to establish and maintain the best individualized home care plan.

We provide services to any eligible client living within 30 miles of Mt. Angel, Oregon. This includes the upper Willamette Valley area from Stayton to Wilsonville and from West Salem to the Cascade foothills. Our services include:

Registered nursing and 24-hour nurse response:

  • Injections/medication management
  • Catheter care/bowel/bladder programs
  • Wound care
  • Comfort measures/pain control
  • Patient/family education

Home health aides:

  • Personal care services
  • Bathing, dressing, hair care
  • Meal preparation
  • Light housekeeping and laundry

Physical Therapy:

  • Transfer training, gait training
  • Pain management
  • Home exercise programs

Occupational Therapy:

  • Activities of daily living (dressing, bathing, cooking, cleaning)
  • Adaptive equipment to permit independence and enhance safety
  • Exercise and motion-based therapy
  • Fitting and fabrication of splints to decrease pain and deformity
  • Positioning for arm, shoulder, hand and contracture

Speech and Language Therapy:

  • Voice disorder treatment
  • Nonverbal communication
  • Swallowing difficulties
  • Auditory training

Medical Social Services:

  • Financial and community resources
  • Skills for coping with illness
  • Short- and long-range planning
  • Decision-making with patient and family
  • Grief and loss counseling
