The school form should be completed by the teacher(s) and counselor who have worked most closely with your child over the last year. Please ask them to complete each question and provide additional comments.
If your child currently has an IEP, we would like you or the school to attach a copy. We would also like you or the school to include as many past report cards as possible, particularly the most current.
If your child has been evaluated in the past by a private psychiatrist/psychologist/therapist, etc., please include copies of their report(s). Also, if your child has had a psychological evaluation at school, we would like a copy enclosed.
In order to understand your child's specific learning needs and develop curriculum to address those needs, we require the following tests:
Weschler Intelligence Scale for Children (WISC-IV)
Weschler Individual Achievement Test (WIAT or WIAT-II)
If your child has been given these tests within the last year, please forward copies to us. If not, we would suggest that you look into scheduling them.
Before our admission committee reviews an application, a prospective student needs to have completed an observation day at Gately Academy. This gives us valuable firsthand information about your child and often lessens his/her anxiety about a possible new school placement. Please schedule this visitation day with our admissions coordinator at 503-215-2672.
Please include a photo of your child with your application.
One final note: If you intend to ask your school district to pay to send your child to Gately Academy, either now or in the future, please contact them before proceeding with this application. Any such placements by the school districts must be initiated by them.
