We provide around-the-clock skills, support and assistance for patients and in-home caregivers. When you or a loved one leaves the hospital but still require skilled nursing or therapies, Providence Home Services is there to help with a wide range of services you can trust and depend upon anytime, day or night. Whether it’s physical or speech therapy, wound care assistance, social work, psychological expertise or nutrition proficiency, our caring and dedicated professionals work diligently with the patient, physicians, family members and other caregivers to achieve optimal health care goals in the comfort and convenience of your home.

Services
Our services are available to patients and families within a 30-mile radius of Providence Portland Medical Center and Providence St. Vincent Medical Center. Patients living outside this service area will be referred to a member of our provider network located closer to their home.
Insurance coverage
Medicare, Medicaid, Worker’s Compensation and most private companies and HMOs cover the cost of Home Health care. Many plans require a prior authorization and conditions vary with each plan. Our staff can help you determine your coverage and payment options and help obtain necessary pre-approvals. Patients are never turned away for financial reasons.
Patient referrals
Patients can be referred by their physicians or can request services directly from Home Health. Direct requests will be coordinated with a patient's personal physician. Please call our referral line at 503-215-4646 (or 800-531-9754 outside the Portland metro area).
