Mental Health Home Care, a division of Providence Home and Community Services, provides skilled clinical services to individuals who need psychiatric services but cannot access traditional outpatient resources due to the nature of their mental illness or physical disability. In addition, medical social work or occupational therapy may be coordinated to achieve optimal goals and outcomes for each patient. Mental Health Home Care can be provided in patients' homes, adult foster care settings, or intermediate care facilities.

Depressive disorders
Bipolar disorders
Schizophrenic disorders
Anxiety disorders
Psychotic disorders
Alzheimer's-type dementia with delusions, delirium or depressed moods
Mental health team
Recognizing that individuals with mental illness often have a variety of needs, we have built a team of mental health specialists who work together to restore health and emotional stability. The team represents various professional specialties. Services from one or all may be ordered by the referring physician. Integral to the team are the following members:
Registered Psychiatric Nurse
Performs initial assessment; develops a comprehensive care plan
Administers and manages medications
Provides behavioral and cognitive therapies
Oversees ongoing management and care plan evaluation
Monitors symptoms, status and disease process
Coordinates other Home and Community Services as needed
Medical Social Worker
Evaluates social and/or emotional factors related to the illness
Determines appropriate level of care; assists with residential placement
Provides referrals to community resources
Occupational Therapist
Assesses rehabilitative potential and needs
Implements therapeutic tasks and activities as part of an occupational therapy program
Evaluates cognitive functioning
Physical therapists, speech therapists and home health aides are also available.
